For inquiries related to Medicare, Disability, Life, or Accident Insurance, the following provisions apply:
- Authorization for Use/Disclosure of PHI
- By submitting information for health-related insurance products, you authorize MI Coverage Pro and its affiliates to collect, use, and disclose Protected Health Information (PHI) as defined by the Health Insurance Portability and Accountability Act (HIPAA), 45 C.F.R. § 160.103, for the sole purpose of:
- Providing insurance quotes
- Determining eligibility for coverage
- Facilitating policy issuance
- Complying with legal obligations
- Limited PHI Sharing
- PHI may be shared only with:
- Licensed insurance carriers
- Third-party administrators (TPAs)
- Government agencies (e.g., CMS for Medicare)
- No PHI will be sold for marketing purposes unrelated to insurance services.
- Consumer Rights Under HIPAA
- You may:
- Request access to your PHI
- Amend incorrect PHI
- Revoke this authorization in writing (except where disclosures have already occurred)

- Medicare Advantage/Part D: Not all plans available in all areas. Carrier-specific restrictions apply.
- Enrollment Periods: Quotes provided do not guarantee eligibility during Annual Enrollment (AEP) or Special Enrollment (SEP) periods.
